Healthcare professionals, please initiate referrals by completing the Referral Form and faxing it to (207) 221-1710.

MaineCare, Medicare, some commercial insurances and self-pay are accepted.


Our relatively small, welcoming agency provides all of the following services at a single, convenient location:

 

BHRM is committed to continually re-evaluating and improving these services to meet the complex challenges facing the persons we serve. Most recently we have developed a more flexible, individualized model of care in the treatment of addictions and co-occurring mental health disorders (COD).

  • We are increasing individualization in our Suboxone program, to reduce barriers to treatment.

  • We are individualizing treatment based on the client’s “stage of change”, providing Motivation Enhancement Treatment (MET) as an alternative to IOP for those persons who are still “contemplative” about their recovery.

  • We offer a special addiction treatment track for persons with more serious co-occurring mental health and substance abuse disorders (COD).

  • We are offering a psychiatric evaluation to our substance use clients who do not currently have a psychiatric provider.

  • We are adding an expanded array of “best practice” outpatient groups including groups for trauma and PTSD, anger management, motivation enhancement, and building cognitive-behavioral skills.

Constance W. Jordan

MSN, ANP, PMHNP

BHRM’s CEO and Clinical Director Constance W. Jordan, MSN, ANP, PMHNP, received her Masters of Science in Nursing from Simmons College and her Post Masters Certificate from Rush University. She is Board Certified as an Adult Nurse Practitioner and as a Psychiatric Mental Health Nurse Practitioner. NP Jordan has a unique and extensive blend of experience caring for patients with psychiatric disorders and chronic mental illness, given her critical care and advanced practice background she specializes in psychopharmacology/medication management for individuals with psychiatric illness and co-morbid medical disorders. She provides psychiatric evaluations, psychopharmacological medication management, mental health consults and educational lectures regarding psychiatric illness and mental health care.

Christina Markoulis

LMSW-CC, CADC

Christina Markoulis LMSW-cc, CADC is the Clinical Program Manager for BHRM. She graduated fro University of New England with a Master’s in Social Work. She has worked primarily with co-occurring disorders, specifically substance abuse and mental health; in her professional career she has worked with individuals who are incarcerated and is an advocate for treatment as opposed to incarceration.

Christina is passionate about incorporating social justice in the social work field and primarily utilizes Motivational Interviewing and Rational Emotive Behavioral Therapy with her clients.

Peter Wohl

MA, LADC, CCS

Peter Wohl, MA, LADC, CCS is BHRM’s Director of Outpatient Services and COO. Peter’s career in behavioral health services spans over two decades. Prior to coming to BHRM he was the Director of Outpatient Services for Crisis & Counseling Centers, Inc. (C & C) in August Maine. C & C was identified as one of three centers of excellence in Maine for the treatment of co-occurring disorders. His 15-year tenure with that agency began with consulting on co-occurring disorders (COD) at Augusta Mental Health Institute (AMHI). He then started a co-occurring IOP, one of the first in the state. Prior to becoming Director of Outpatient Services, he served as the Director of Substance Abuse Services. Those services grew to include four IOP programs and fourteen outpatient clinicians at their peak.

Linnea Purcell

LMSW-cc, LADC

Linnea Purcell, LMSW-CC, LADC joined BHRM in the spring of 2015. She currently runs IOP groups, Aftercare Groups and Anger Management Group. Her approach is client-centered and strength-based with an emphasis on establishing an empathetic, safe therapeutic alliance. Ms. Purcell offers extensive knowledge and training in addiction, she works with adults individually and in group setting who are impacted by serious co-occurring conditions, including a variety of mood, and anxiety disorder’s, as well as trauma, abuse, addiction, grief and loss, stress, probation, life transition and relationship conflict.

Linnea is trained in Motivational Interviewing, Acceptance and Commitment Therapy, Cognitive Behavioral Therapy. She holds a Master’s degree in Social Work from the University of New England, in Portland, Maine with a clinical concentration.

Krystle N. Ladd

MSN, PMHNP

Krystle N. Ladd, MSN, PMHNP is a board certified Psychiatric Mental Heath Nurse Practitioner who received her Master of Nursing Practice from University of South Alabama. Krystle is currently in the process of obtaining her Doctor of Nursing Practice with an emphasis on psychiatric medication adherence. Her nursing career began providing case management services to individuals with developmental disabilities and co-occurring mental health needs. She was the Assistant Director of Nursing at a mental health residential facility before returning to school to become a Psychiatric Mental Health Nurse Practitioner.

Ellen Farnsworth

MSN, MEd, ANP-BC, NP-C

Ellen Farnsworth, MSN, MEd, ANP-BC, NP-C, received her Master Degree in Health Education from Temple University, Philadelphia and a Master Degree in Nursing from Simmons College, Boston. She is Board Certified as an Adult Nurse Practitioner from the American Nurses Credentialing Center and the American Academy of Nurse Practitioners. Her experience as a Nurse Practitioner has been in Primary Care, College Health, Women’s Health, Homeless Health, Occupational Health and Addiction Medicine in both Methadone and Suboxone settings.

She has been a former President of the Maine Nurse Practitioner Association and the Maine College Health Association. She is a member of Sigma Theta Tau and the American Society of Addiction Medicine. Hobbies include hiking, Yoga, cooking, reading, and family activities.

My philosophy of practice is based on facilitating empowerment in my patients to be partners in their health care. My joy comes from patients succeeding in their journey to recovery.

Susan Martin

MS, NP

Susan Martin, MS, NP, received her B.S.N. from Northeastern University in 1980 and her Adult NP from Boston University in 1986. She did a post masters in Family Practice at University of Southern Maine and graduated in 2004. She has worked with under-served populations for many years of her career at various facilities including jails and prisons, methadone treatment, an HIV practice in Baltimore, and provided primary health care in FQHC. She enjoys working with challenging populations and looks to support patients trying to make a change in their life. She enjoys traveling, movies, and relaxing at home with her husband and three cats.
